This project was a collaborative effort with the North Georgia Health System (NGHS).  Using recycled tennis cans, “Plan in a Can” is designed to provide Hall County residents with an inexpensive, functional tool to improve the transition of patient care in emergency situations and to improve communication between patients, relatives and medical staff.

After consulting with emergency and medical personnel, project team members developed an identifying can label, English/Spanish medical insert forms, and a tennis can recycling/collection system throughout Hall County and beyond.  From September to December 2019, team members also tested the program at various locations, conducted ten (10) presentations, and participated in three health care expos.

The project was formally adopted and integrated into the NGHS Advance Directive Program in January 2020.  To date, over  3000 cans have been collected and the recycling and presentation program will continue with NGHS volunteers.
